All patients referred (apart from COVID-19 patients) should not be sent home until someone from the Virtual Acute Care Unit has seen the patient to set them up with monitoring equipment unless otherwise agreed with a member of the team.
FOR NON-COVID PATIENTS (referrals from 9am to 4pm only) – all referrals should be made by calling 8104 and not via an online order until it has been discussed with one of the team on the phone. Referrals must be approved by a named medical consultant.
FOR COVID PATIENTS – Refer via adding an order on EPR for “AMU Virtual Ward (COVID)”. Please ensure you have confirmed that the patient contact details are correct on EPR or if not, that they are documented in the notes.
